A new study rewrites the currently popular belief that injection drug users only turn to oxycodone, OxyContin, codeine and other prescription painkillers as substitute drugs, long after they have started injecting heroin.
In fact, a study at Drexel University's School of Public Health has found four out of five young injection drug users abused oxycodone, OxyContin or another prescription opioid painkiller, long before stepping up to injecting heroin or morphine.
Not only that, the study has found that the first drug injected by one out of every four young injection drug users was oxycodone, OxyContin or another prescription painkiller, rather than the drugs usually associated with injection. Oxycodone is the most commonly abused opioid prescription painkiller, is the active ingredient in OxyContin, and can be dissolved and injected like heroin.

Young injection drug users
Prescription opioid misuse is a widespread and deadly public health concern. In Florida, oxycodone abuse is the leading cause of fatal drug overdoses. Studies have looked at the extent of prescription drug abuse, but little is known about how and why young drug abusers start injecting.
The Drexel University study, published in the International Journal of Drug Policy, explored the factors surrounding young injection drug users' initiation into the misuse of opioids. The study took place during 2008 and 2009.
The study found that nearly all young injection drug users in the study:
- Had family histories of substance abuse (drugs and alcohol were “normal”); witnessed family members misuse substances during childhood and adolescence ranging from alcoholism to injecting heroin
- Had been prescribed opioid painkillers in mid-teens
- Were pressured by friends or attracted by financial incentives to sell available prescription opioid painkillers
- Never completed high school, were expelled or held back a grade
- Wanted to experiment with opioids
- Escalated from prescription opioid misuse to injection abuse
- Were homeless at some point, most currently homeless
- Had received psychological diagnoses such as depression, anxiety, or Attention Deficit Hyperactivity Disorder (ADHD), with a history of psychoactive drug treatments
- Regarded prescription opioids like oxycodone and OxyContin as valued commodities to trade or sell
